Minnesota requires all students enrolled in grades kindergarten through 12 to show they have received immunizations or an exemption. Legal exemptions to the MN State law are permitted. These include exemptions for medical reasons or for conscientious beliefs of the parent or legal guardian. The required form for exemptions must completed by parent or MD, when necessary and must be notarized.

Immunizations are only available in the School Based Clinics for students who meet one of these criteria: student/guardian has no insurance, student/guardian has Medical Assistance, or student is Native American.
