Vaccine Clinics for MPS Students Ages 12+

Vaccine event

The Washburn School Based Clinic, in partnership with MPS, Hennepin Health, and the Minneapolis Health Department will be hosting vaccine events at Washburn High School for all MPS students ages 12+.  For students ages 12-15, a parent or guardian must accompany the minor for vaccination. The accompanying guardian/parent must be at least 21 years of age.

Vaccine: Pfizer
Where: Washburn High School Main Gym, Enter through Door 5A
Who: MPS students, ages 12+. We may open appointments to families if spots are still available. Check back here for details later in the week.

AGE 16 – 18:  Parent may verbally consent over the phone with the vaccinating nurse if they are not present.  Nurse will write on the Registration Form verbal consent given with the name of the guardian/parent and the nurse’s initials in the parent/guardian signature line.
AGE 12 – 15: A parent or guardian must accompany the minor for vaccination. The accompanying guardian/parent must be at least 21 years of age.

HOW TO REGISTER

Complete consent and registration forms found at the links below. Paper copies also available in main office, Room 005 (School nurse) or Room 002 (School Based Clinic), or ask a teacher for help printing. Note: ALL students younger than 18 years old MUST have signed parent/guardian consent prior to vaccination. Vaccinations cannot be given without parent/guardian consent.

Reminders: 

  • You MUST schedule a vaccine appointment time, no walk-in's allowed
  • Please wear a mask and respect social distancing signs
  • Parents MUST sign the registration form if your student is under 18 years old. For students ages 16-17 you may send the student with the signed and completed registration form or you may accompany your student to the appointment. Registration forms will also be available on-site. Vaccination will not be given if registration forms are not completed by a parent/guardian.
  • For students ages 12-15, a parent or guardian must accompany the minor for vaccination. The accompanying guardian/parent must be at least 21 years of age. 
  • Spanish and Somali language interpreters will be available on-site.
